FACTS Grant & Aid Assessment is processing all applications and conducting the financial need analysis for School Choice Scholarships. Families applying for scholarships must complete an application and submit the necessary supporting documentation to FACTS on or before February 28, 2019.
Once an online application has been completed, the following information must be sent to FACTS to complete your application:
Copies of your 2017 Federal tax return including all supporting tax schedules & copies of your 2017 W-2 forms.
Copies of Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, and TANF, where applicable.
Supporting documents can be submitted one of three ways:
1. ONLINE by uploading in pdf format

All eligible applicants will be included in our lottery draw in May 2019 to determine scholarship awards for the 2019-20 school year. All applicants will be notified by June 30 whether or not they have received a scholarship. Applicants not chosen in the lottery will be placed on our waiting list and notified if a scholarship becomes available.
In order to apply for a scholarship a child must:
Live and attend school in Jefferson or Oldham County
Be entering Kindergarten (K) through the Sixth Grade (6) in the Fall of 2019 at any private or parochial school (Unless your child is currently in the program, in which case you can apply through the eighth grade).
Be Eligible for the Free and Reduced Lunch Program
